Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 03/23/2022 and concluded on (04/06/2022. The director was contacted by telephone to initiate the inspection. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center did note ensure all areas/equipment of the center were maintained in a safe condition.
Evidence:
1. In the 3 year-old room there was peeling paint on the walls in reach of children.
2. In the school-age room there was a tall wooden bookshelf that was not secured, posing a tipping hazard.
3. In the school-age room the vinyl on the booth seating was torn and the inside was exposed.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The Director will contact maintenance and request for peeling paint removed, book shelf secured and the vinyl will be repaired or discarded.
