Today an unannounced Monitoring Inspection took place between the hours of 2:45pm and 4:20pm. There were 2 rooms observed with a total of 29 children with 4 staff within the supervision guidelines. The children were observed arriving from school on the programs buses, eating snack, playing in the gym and doing homework. A complete inspection of the physical plant, children and staff records, emergency procedures were observed during this inspection.

Standard #: 8VAC20-780-160-C Description: Based on record review and interview, staff members have not submitted the results of a follow-up tuberculosis (TB) screening at least every two years.
Evidence: Staff #1 most recent TB screening is dated 7/3/2019. Staff #1 confirmed that they have not completed a follow-up TB screening. This was previously cited at the programs last inspection on 12/13/2021.Plan of Correction: Staff #1 went to complete an updated TB screening.
Standard #: 8VAC20-780-40-N Description: Based on interview, the center has not developed written playground safety procedures.
Evidence: The program director stated that playground procedures have not yet been established. This was previously cited at the programs last inspection on 12/13/2021.Plan of Correction: Write up the safety procedures and have them posted in classroom for staff to see and understand how to follow and make sure they are set in place.
Standard #: 8VAC20-780-245-A Description: Based on record review and interview, staff have not completed 16 hours of training annually.
Evidence: Staff #1 and Staff #2's records do not contain documentation of any training completed in 2021. The program director confirmed that the staff did not complete any training in 2021.Plan of Correction: Connect with both staff and find sites or online training to complete 16 hours of annual training.
Standard #: 8VAC20-780-245-L Description: Based on interview, there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence: The program director confirmed that none of the staff have completed daily health observation training. This was previously cited at the programs last inspection on 12/13/2021.Plan of Correction: Getting staff to complete the health observation training.
Standard #: 8VAC20-780-340-F Description: Based on observation and interview, children under 10 years of age were not always within sight and sound supervision of staff.
Evidence:
1. At the start of the inspection, large groups of children were observed running off the centers buses and into the building by themselves. The children were observed lingering in the hallways around the bathrooms and lockers before entering the gym area, where the staff were located.
2. Children were observed freely running back and forth from the classrooms to the bathrooms without any supervision from staff.
3. Children were frequently observed pacing back and forth in the hallways between the classroom, gym and bathrooms while the staff were in the classrooms.
4. There are no systems to assure that individuals who are not staff members or persons allowed to pick up a child in care do not enter the public restroom area while in use by children.Plan of Correction: Make sure staff are waiting at the door to pick up the children and take them to the gym. Have everything more organized. Take the children to the bathrooms so it's safe and monitored at all times.
Standard #: 8VAC20-780-530-A-2 Description: Based on record review and interview, there was not at least one person present with current first aid training.
Evidence:
1. Staff #1's first aid training expired 5/10/2021.
2. Staff #2, Staff #3 and Staff #4 have not yet completed first aid training.
This was previously cited at the programs last inspection on 12/13/2021.Plan of Correction: Staff #1, #2, and #4 completed their first aid training 3/29/2022. Staff #3 needs to complete.
Standard #: 8VAC20-780-560-F Description: Based on observation, a food menu was not available when food was served.
Evidence: The children were observed eating a snack served by the staff. The program director stated that they do not have a food menu.Plan of Correction: Weekly schedule made that includes snack to be placed on classroom bulletin board.
