This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.
A monitoring inspection was conducted on 04/08/2022 and a virtual record review was conducted on 04/07/2022. The inspection was conducted via Zoom. There were 34 children, ages 6 years-12 years, and 4 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-B-4 Description: Based on record review and discussion with staff, the center failed to obtain a copy of a child abuse and neglect search and a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 2 contained documentation that Staff 2 had resided outside of the state of Virginia within the preceding five years. There was not a child abuse and neglect search nor a sex offender registry check from all of the states in which the individual had resided in the preceding five years available for review during the 04/07/2022 inspection. The documented hire date in the record for Staff 2 was 01/10/2022.Plan of Correction: Will request the information.
