Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A subsidy health and safety (SHSI) inspection was initiated on April 5, 2022 and concluded on April 5, 2022. There were 30 children present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #:	8VAC20-790-600-D-1
Description:	Based on a review of staff records and interview on April 5, 2022, the vendor did not obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of the children in care within 90 days of employment. Evidence: 1.) The record of staff #1 (hired 11/22/21) contained documentation of completion of CPR dated 3/18/22. 2) The record of staff #2 (hired 1/5/22) did not have documentation of certification in CPR. 3.) Administration acknowledged the certifications were late and not completed.
Plan of Correction:	Director has contacted new CPR group who will be involved in the new hire process for certifying staff on time.
Standard #:	8VAC20-790-600-D-2
Description:	Based on a review of staff records and interview on April 5, 2022, the vendor did not ensure each staff member had a current certification in first aid within 90 days of employment. Evidence: 1.) The record of staff #1 (hired 11/22/21) contained documentation of completion of first aid dated 3/18/22. 2) The record of staff #2 (hired 1/5/22) did not have documentation of first aid certification. 3.) Administration acknowledged the certifications were late and not completed.

Standard #:	8VAC20-790-600-F
Description:	Based on a review of staff records and interview on April 5, 2022, the vendor did not ensure that each staff complete the Virginia Annual Health and Safety update training annually. Evidence: The record of staff #3 (hired 9/3/20) did not contain documentation of the annual health and safety update training. Documentation of the Virginia Pre-Service training is dated 7/2/20. Administration confirmed the annual update was not completed.
Plan of Correction:	Staff will change the documentation process to make sure all certifications are on file.
