Discussed written protocol for all programs related to care and protection of children who incur head injury while in care

Comments:
An unannounced serious injury investigation initiated based on a self reported incident. An investigation was initiated on March 7, 2022 in response to an allegation made regarding care and protection of children in care and was concluded on March 25, 2022. Based on staff interviews and the review of documentation, the preponderance of evidence gathered during the investigation supports the allegation. The allegation is determined to be valid. Standard #: 8VAC20-780-340-A
Description: Based on staff interviews, when staff were supervising children, staff failed to ensure their care protection and guidance.

Evidence:
There were 3 staff supervising and 11 children present.
1. On 3/4/2022, Staff A observed Child A sustain an injury to the head after hitting her head on a piece of playground equipment. Staff A is unsure of the time, she stated that she thinks it was after 1:00 pm.
2. Staff A said that she asked Child A if she was okay and Child A said that she was okay and returned to play.
3. Staff A said that she did not check Child A for an injury. No first aid was rendered.
4. At approximately 1:30 pm, Staff A and Staff B took the children back inside. Staff A was in front of the line and Staff B was at the back of the lie and Staff C was on break.
5. When they got back inside the building, Child B begin coughing and having difficulty breathing. They sat the other children down and called Staff C.
6. As they waited for Staff C, (Staff C was on break in her car in the school parking lot). Staff C arrived within minutes and contacted Child B's parent, who arrived with medication.
7. After Child B's parent had arrived and had taken care of Child B. Staff C noticed that Child A was holding her head and Staff C asked Staff A said that "she had forgotten about Child A, and that the child had hit her head outside"
8. Staff B got Child A an ice pack.
9. Staff C walked over to Child A. Staff C examined Child A's head and felt and saw a "goose egg" and asked the child how she felt? The child stated "my head hurts".
10. Staff C walked with the child over to the couch area. Staff C then called the parent and explained that the child had hit her head, it was between 2:00pm-2:15pm. Staff C informed the parent that the child also stated that she had a headache. The mother stated that the child was "probably fine" and asked Staff C if the child had fallen asleep or had nausea. The mom also spoke to the child. Mon stated that she was in Bethesda, but would get there as soon as possible. Mom also stated that she would contact dad.
11. Staff C then contacted the Regional Director to report the incident.
12. Staff C then went back to the child and asked how she was feeling, the child stated "my head hurts and my face hurts." Staff C noticed that the child's eyes were unfocused and her hands and her body started to convulse.
13. Staff C begin to render first aid and called 911, she asked the staff to move the other children to the other room, she also asked the staff to contact the office and let them know that an ambulance would be arriving.
14. Staff C contacted the mother and advised that the child had had a seizure, the mother asked that the child be taken to INOVA-Fairfax Hospital.
15. Staff C accompanied the child to the hospital and met the mother there. Child was diagnosed with having had a contact seizure and a concussion.

Plan of Correction: Staff will ensure the care, protection and guidance of all children while in care. Staff will participate in training which will include a review of program policies for active supervision, responding to emergencies and first aid procedures. Staff will also review center specific outdoor safety procedures, including the Playground Safety Plan and Injury Prevention Plan. Additional procedures for prevention of head injuries and ensure the care and protection of children who incur a head injury white in care willbe shared with all satff and updated in the program operations manual.

Description: Based on interviews, the facility failed to notify parents immediately of a serious injury sustained while in care.

Evidence:
On 3/4/2022 Child A sustained a head injury on the playground at approximately 1:00 pm, the supervisor was not notified of the child's injury until approximately 2:00 pm and the parent was notified at 2:15pm

Plan of Correction: Staff will ensure parents are notified immediately of any head or serious injury. Staff will participate in training which will include a review of program policies for active supervision, responding to emergencies and first aid procedures
