An unannounced complaint investigation was initiated on 3/14/22 and was concluded on 4/6/22 in response to a complaint received by the licensing office on 3/14/22 regarding supervision and procedures for emergencies. Observation, record reviews, video footage review, and interviews were conducted relating to the allegations on 3/14/22 and 4/6/22. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based on video footage, record review and interviews it was determined the staff did not ensure the care and protection of the children they were supervising at all times.
Evidence:
1. On March 10th, 2022 at approximately 2:40 pm Staff #1 tilted a table with one hand while vacuuming with the other in close proximity to a group of 5 preschool aged children seated on the carpet.
2. Video shows the table slip from Staff #1's hand falling onto child #1(20 months old) hitting them in the face and stomach.
3. Video footage shows Staff #1 slowly approach child #1 and then proceed to comfort child, assess and treat for injury.
4. Injury report dated 3/10/22 confirmed the the table slipped out of staff #1's hand and fell on child #1 but did not report the nose bleed.
5. Parent reported that upon taking child to Urgent Care dried blood in their nose and a busted top lip. Parent contacted the staff who confirmed the nose did bleed.Plan of Correction: -

Simply slide the table to gain access to clean underneath and to make sure children are protected & supervised at all times while doing so. Staff #1 is no longer an employee based upon this incident and not following proper operating procedures.

Evidence:
1. The Injury report dated 3/10/22 for child #1 did not describe the type of the injury.
2. Staff reported a table fell over on top of child #1, staff noted "the table appeared to fall on the child's stomach but there was discoloration of the nose and liquidly blood on the child's face". These details were not captured on the injury report.
3. Staff was seen in the video cleaning the inside of child #1's nose, this was not documented on the injury report.Plan of Correction: Staff will be retrained in the area of injury report writing as to report every thing they see and transfer that information to the injury report as to not omit very important information. Staff #1 is no longer employed for violating this operating procedure.
