22.1 Early Childhood Care and Education
An unannounced monitoring inspection was conducted on 3/23/22 from 10:00am to 11:50am with the Director. There were 21 children in care, ranging in age from six-months to five-years-old, supervised by four staff. The children were observed finishing morning snack, singing songs, and playing on the playground. Three child records and two staff records were reviewed. The Center reports that all staff have current certification in CPR and First Aid as well as Daily Health Observation Training. There were no medications on site for children. The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 7/26/21 and the most recent Health Inspection on file was dated 2/17/22. Standard #: 22.1-289.035-B-2 Description: Based on review of two staff records, the center did not obtain documentation of the results of a national fingerprint-based criminal background check prior to date of hire for one staff. Evidence: The fingerprint-based criminal background check on record for Staff B, with a start date of 12/13/21, was dated 12/20/21. Plan of Correction: We did obtain documentation for everything prior to the hire date however the wrong start date was used inadvertently.
Standard #: 22.1-289.035-B-4 Description: Based on review of staff records, the center did not obtain the results of a sex offender registry check or central registry check from all states in which current staff members have resided within the past five years by December 31, 2020, as required. Evidence: The driver's license on record for Staff A documented a Maryland home address and the sworn statement signed by Staff A on 12/14/18 indicated that Staff A had lived in Maryland within the past five years. There were no out-of-state background checks for Maryland on record for Staff A. Plan of Correction: We did mail out the paperwork to Maryland which was returned for a missing middle name. The employee does not have a middle name. The paperwork was mailed back to Maryland. When we called to check on the status we where told that they could not find the returned paperwork so we have sent new paperwork.
Standard #: 8VAC20-780-160-A-1 Description: Based on review of two staff records, the center did not ensure that all staff submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The TB screening on record for Staff B, with a start date of 12/13/21, was dated 12/19/21. Plan of Correction: Staff start date was after TB test was done. An incorrect start date was used.
Standard #: 8VAC20-780-270-A Description: Based on observation, equipment of the center was not maintained in a safe condition. Evidence: On the playground there was a plastic play house that had a large cracked piece on the windowsill area that created a sharp-edged hole which posed a scratching hazard. Plan of Correction: The play equipment is being removed from the playground.
Standard #: 8VAC20-780-560-F Description: Based on observation and interview with the provider, the center did not ensure that a menu listing foods to be served for meals and snacks during the current one-week period was posted or given to parents. Evidence: The menu posted inside the center and on the front door for parents was dated "February". Plan of Correction: The updated menu has been posted.
