Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on 04/07/2022 and concluded on 04/07/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 21 children present, ranging in ages from 5 years to 10 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed/updated.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on interview and children's record review, the center failed to ensure that all required information was obtained in and in the children's files.

Evidence:

1. Child #2's file did not contain two emergency contacts that were different from the guardian/parents.

Plan of Correction: I will contact the parents and get two emergency contacts that are not them.
