Comments:
An unannounced subsidy health and safety (SHSI) inspection was initiated on 03/15/2022 and concluded on 03/30/2022. There were 99 children present with seven staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-580-B
Description: Based on a review of eight staff records, the vendor did not ensure three staff members completed the Virginia Preservice Training for Child Care Staff within 90 days from subsidy vendor approval.

Evidence: 1) The Virginia Preservice Training certificate for Staff #3, hired on 09/25/20, was dated 03/29/22.

2) The record for Staff #5, hired on 09/29/17, did not contain a Virginia Preservice Training certificate.

3) The record for Staff #6, hired on 11/09/21, did not contain a Virginia Preservice Training certificate.

4) The vendor became an approved subsidy vendor on 12/16/21 and the training should have been completed no later than 03/16/22.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of eight staff records, the vendor did not ensure three staff members had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days from subsidy vendor approval.

Evidence: 1) The record for Staff #1, hired on 08/31/18, had a CPR certification that expired on 11/05/21.

2) The CPR certification in the record for Staff #5, hired on 09/29/17, was dated 03/22/22. The previous certification expired on 07/26/20.

3) The CPR certification in the record for Staff #8, hired on 09/29/17, was dated 03/29/22. The previous certification expired on 11/05/21.

4) The vendor became an approved subsidy vendor on 12/16/21 and the certification should have been completed no later than 03/16/22.
