Comments:
A renewal inspection was completed on 4/20/2022. There were 42 children in care, with a total of 8 staff. The Inspector reviewed compliance in the areas of administrative records, background checks, emergency preparedness, the indoor and outdoor physical plant, children's medications, and programming. A total of 6 children's records/medications were reviewed, and 5 staff records.

The children were observed engaging in creative play activities, working on art projects, and enjoying circle time discussions.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, it was determined that two records do not contain documentation of confirmation of up-to-date information.
Evidence:
The records for child D and child F do not include documentation of an annual review completed by the parent. The record for child D was reviewed by the parent on 2/2/2021; the record for child F was reviewed by the parent on 2/24/2021.

Plan of Correction: Both parents will be asked to document a review of the contact information. We will also create a system to have all parents review their child's record on the same day.

Standard #: 8VAC20-780-330-B
Description: Based on observations, it was determined that in various fall zones on the playground the level of resilient surfacing does not comply with minimum safety standards.
Evidence:
In the following fall zone areas, the level of mulch has worn down to dirt: the slides, the climbers, and the stationary riding pieces.

Plan of Correction: Since the school year is almost over we will rake the mulch for now and I will increase the mulch deliveries starting next school year.

Standard #: 8VAC20-780-510-G
Description: Based on a review of children's medications, it was determined that one non-prescription medication was not labeled as required.
Evidence:
The medication for child B was not labeled with the child's name and dosage instructions.

Plan of Correction: The medication will be labeled today.
