Technical Assistance:
The new Standards for Licensed Child Day Centers is available on the Virginia Department of Education website.

Comments:
An unannounced monitoring inspection was conducted today. There were 45 children present with 13 staff supervising. The physical plant, children's records, staff records, evacuation logs, menu's, cribs, bottles and baby food and first aid kits were reviewed. The children were observed playing games and working in small groups, they were also observed in the playroom. Areas of non-compliance were cited on the Violation Notice. Standard #: 8VAC20-770-60-B
Description: Based on a review of staff records, the center failed to ensure that an employee of a licensed child day program must not be employed until the agency has the person's completed sworn statement or affirmation.

Evidence:
Staff #5's record does not have documentation of a completed sworn statement or affirmation. Date of hire 03/28/2022.

Plan of Correction: Sworn statement was signed and placed in file. The Director will make sure every form is signed on the first day of employment.

Standard #: 8VAC20-780-130-A
Description: Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #2's record does not have documentation of immunizations before or on the first day of attendance. First Date of Attendance - 06/20/2020.

Plan of Correction: Director will reach out to parents for immunization verification.

Standard #: 8VAC20-780-140-A
Description: Based on a review of children's records, the center failed to ensure that each child shall have a physical examination within 30 days after the first day of attendance.

Evidence:
Child #2's record does not have documentation of a physical by the 30th day of attendance. First Date of Attendance - 06/02/2020.

Plan of Correction: Director will ensure that every child has a physical examination within 30 days of enrollment.

Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center failed to ensue that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
Staff #4's tuberculosis test expired 02/24/2022.

Plan of Correction: Staff with expired TB test or screen will get updated TB test. and submit records to center.

All staff TB test will be reviewed.

Description: Based on a review of children's records, the center failed to maintain and keep at the center the required documentation for each child enrolled.

Evidence:
Child # 2's record does not have documentation of a physician's name and phone number/
Child #1 and #2's records do not have documentation of names of persons authorized to pick up the child.
Child #2, #4 and #5's records do not have documentation of viewing proof of the child's identity and age.

Plan of Correction: Director reached out to parents and will gather all required information

Description: Based on a review of staff records, the center failed to ensure that required records were obtained and maintained at the center.

Evidence:
Staff #1, #2, #3, #4 and #5's records do not have documentation of emergency contacts.
Staff #5's record does not have documentation of two reference checks.

Plan of Correction: Staff's emergency contacts were gathered immediately.

Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the Three Year Old room, there was diaper cream, marked keep out of reach on a low shelf.
In the Four Year Old room, there were Clorox Wipes and Lysol on a shelf.
In the Playroom/Nap Room, there was a bottle of hand sanitizer on the book shelf.

Plan of Correction: Corrected during inspection.

Standard #: 8VAC20-780-440-L
Description: Based on observation, the center failed to ensure that comforters shall not be used by children under two years of age.

Evidence:
In the Infant room - ages 4 months - 12 months old, Child #A was observed with a large blanket while sleeping in his crib.

Plan of Correction: Blankets was removed from cribs immediately and returned to parents. Center will send out email related to Safe Sleep information.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
In the Infant room, there were eggs and sweet potatoes in containers in the refrigerator; however, they did not have any label's on them.

Plan of Correction: Food containers were labeled and staff was reminded to have parents label containers and bottles each day.

Standard #: 8VAC20-780-570-B
Description: Based on observation, the center failed to ensure that bottle fed infants who cannot hold their own bottles shall be held when fed.

Evidence:
In the Infant room (4 months - 12 months) an infant was observed lying on their back and not being held during a feeding.

Plan of Correction: Staff was reminded to hold infants during feeding. A sign was also posted in the Infant Room.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center failed to ensure that prepared formula shall be refrigerated, dated and labeled with the child's name.

Evidence:
In the Infant room, in the refrigerator there were four bottles without labels and there was one bottled on the counter without a label.

Plan of Correction: Bottles were labeled immediately. We will remind parents to label the bottles with name and date. Staff was remined to check every bottle in the morning and if bottles are not labeled, they are to be labeled by staff.
