An unannounced complaint investigation was conducted on 04/06/2022 from approximately 11:20 am to 1:45 pm in response to a complaint received by the licensing office on 03/29/2022, regarding staff to children ratios, building maintenance and emergency procedures.

Observations were made in the center, records and policies were reviewed, and interviews with staff were conducted. Based on the information gathered during the investigation, the preponderance of evidence supports the allegations in regards to emergency procedures; therefore, the complaint was determined to be partially valid. The complaint related violations are identified on the Violation Notice.
