- Technical Assistance:
Discussed the need to update classroom bulletin boards and rotate projects with the week at a glance lesson plans to coincide with the lesson.
Discussed improved ways to store snacks that are perishable.
An unannounced monitoring inspection was conducted this date. A total of 82 children with 17 staff were onsite. Walk thru of 4 classrooms was conducted, staff observations were made, review of medications and a total of 3 staff and 3 children's files were reviewed. Violations determined during the inspection. Standard #: 8VAC20-780-40-E Description: Based on review of medications onsite, the Center failed to comply with their own medication policy.
1. The Center's medication policy requires methods to prevent outdated medication.
2. Medication for child #6 expired in 10/2021 and medication for child #7 expired in 2/2021.
Plan of Correction: New medication was requested at pick-up, and all medication is now up to date.
Standard #: 8VAC20-780-340-A Description: Based on observation of the Beginners 2 classroom, staff failed to ensure the
care, protection and guidance for one child.
1. All 3 Staff in the classroom were observed either sweeping the floor, laying out cots for naptime and toileting children.
2. One child was observed finishing their lunch in the dark during this time. The child was observed sitting backwards in their chair and hanging off one side of the chair with food in their mouths.
Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-360-C Description: Based on observation of the Beginners 2 classroom, one child was not allowed to sleep as needed.
1. At the beginning of nap time, a child had fallen asleep on their cot while staff were assisting the children with toileting.
2. Staff were observed waking the child to change their pull up. The child was observed awakened from their sleep and began to cry.
Plan of Correction: Following the visit, Beginner Classroom Teachers were retrained on our sleep policies. We also adjusted lunchtime to start 15 minutes earlier to allow for more time to complete lunch and bathroom visits prior to students falling asleep.
Standard #: 8VAC20-780-370-1 Description: Based on observation of the Infant 1 classroom, an infant was not placed on their back when placed in the crib.
Staff #1 was observed placing Child #3 in a sleep sack and laying Child #3 in a crib on their stomach to sleep.
Plan of Correction: Staff #1 was retrained on Safe Sleep Practices immediately following the visit and signed-of on the policy again. All other Infant room teachers were also retrained. We will continue to train all teachers in the school on safe sleep practices as part of onboarding and as part of our annual training topics.
Standard #: 8VAC20-780-500-A Description: Based on observation of the Infant 1 classroom and the Beginners 1 classroom, staff and one child did not wash hands after toileting and a diaper change.
1. Staff #1 was observed diapering an infant and failed to wash hands after the diaper change.
2. Staff #1 was observed touching an infant bottle directly after changing a diaper without washing their hands.
3. Staff #2 was observed toileting 4 children without washing hands before and after each toileting.
4. Staff #2 was observed entering the toileting schedule into an electronic device in between each toileting.
5. One child was instructed to wash their hands after toileting. The child was observed standing on a stool and proceeded to rinse their hands. The child was unable to reach the soap dispenser. The child was observed drying their hands and was directed to lay down on their cot. The Licensing Inspector informed staff that the child was not able to reach the soap dispenser to effectively wash their hands.
Plan of Correction: Immediately following the visit, all school staff was retrained on proper handwashing for staff and students. The Beginner Classroom stool by the sink was replaced with one with 2 steps to better aid students in reaching the soap. Moving forward, teachers will assist one student in the rest room, then step out to wash his/her hands and ensure the student is also washing his/her hands fully. Infant classroom teachers will remove their gloves then move to the sink with the infant to wash student and teacher hands together before returning to the changing table to clean and sanitize.
Standard #: 8VAC20-780-500-B Description: Based on observation, Staff did not clean and sanitize the diapering surface after each use.
1. Staff #1 was observed changing an infant's diaper.
2. Staff #1 did not return to the diapering area and clean and sanitize the diapering surface.
Plan of Correction: Staff #1 was retrained on our 2- Step Cleaning and Sanitizing Practices immediately following the visit and signed-of on the policy again. All other teachers with diaper changing tables were also retrained. Moving forward, per our policy, all changing mats will continue to be covered with one-time-use changing table paper. This will continue to be removed after each student is changed and mats will be cleaned with a soapy water solution, wiped dry, then sprayed with a bleach solution.
Standard #: 8VAC20-780-510-I Description: Based on review of medications onsite, one prescribed medication was not labeled with the pharmacy label.
Emergency medication for Child # 4 did not have the pharmacy issued label for this medication.
Plan of Correction: The family was made aware the day before the visit of the need for the prescription label but was unable to obtain the label from a pharmacy as it was ordered through the mail. As it was emergency medication, the family was given 2 days to provide a proper label. If they are unable to provide the proper label by Monday, then they have been advised that they are unable to return until it can be provided.
Standard #: 8VAC20-780-510-N Description: Based on review of the Center's medication administration log, documentation of one administration was incomplete.
Medication was administered on 10/28/2021 to Child #5. The name of the staff administering the medication was not documented.
Plan of Correction: Following the visit, all MAT certified staff were retrained on proper completing of medication logs. Moving forward, the forms will be signed and filled out completely.
Standard #: 8VAC20-780-560-G Description: Based on observation of the Infant 1 classroom, food brought from home was not properly labeled.
1. Food brought from home for Child #1 was not labeled with the child's name and date.
2. Food brought from home for Child #2 was not labeled with the date.
Plan of Correction: A reminder email was sent out to all families in the Infant Classroom with a reminder about labeling all food and bottles with a full name and date. Our Infant Classroom Teachers were retrained about proper labeling of food and bottles. Moving forward, all teachers will confirm proper labeling prior to placing food and bottles into the classroom refrigerator and provide reminders to parents as needed.
Standard #: 8VAC20-780-560-L Description: Based on observation of the Beginner 2 classroom, staff did not sit with a child during meal time.
1. All 3 Staff in the classroom were observed either sweeping the floor, laying out cots for naptime and toileting children while one child was observed eating their lunch during this time.
Plan of Correction: Following the visit, all school staff were retrained on the importance of sitting with students as they eat to monitor their safety. Moving forward, if a student arrives after the conclusion of lunch the child will either sit with an administrator or a classroom teacher until the student finishes his/her meal.
