- Technical Assistance:
Discussed the injury prevention plan, fire inspection and health inspection. Discussed that a long electrical cord was hanging near the doors and was a safety hazard. Discussed the required documents to post and or update including the summary and violation page.
An unannounced monitoring inspection was conducted today beginning at 2:00pm. At the time of entrance, 22 children were in care with 2 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, medications, 2 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance. The site was clean. The children were observed checking in, lining up to wash hands and playing games. Areas of non-compliance are identified in the violation notice. For any questions regarding this inspection, please contact Stacy.Doyle@doe.virginia.gov or 571-835-0386.
Standard #: 8VAC20-780-40-I Description: Based on interview, the center did not develop written procedures for injury prevention.
1.Staff member #1 stated she did not have an injury prevention plan for the center.
Plan of Correction: The injury prevention plan will be completed for that site within the next 2 weeks.
Standard #: 8VAC20-780-60-A Description: Based on interview, the center did not maintain and keep at the center a separate record for each child enrolled that included the required information.
1. Child #1 had one parent's name, address, phone number, place of employment and work phone number missing.
2. Child #2 did not have the child's previous child day care and schools attended by the child.
Plan of Correction: It was verified that there was no information to be given by the parents to this section of the registration form. I am working towards getting rid of the paper forms and only using online registrations, which requires information in all required boxes unless it is rejected.
Standard #: 8VAC20-780-540-E Description: Based on interview and observation, the center did not have one working, battery-operated radio in each building and did not have one working, battery-operated flashlight on each floor of each building as required.
1. The center had a radio and flashlight, but did not work as they did not have batteries during the inspection.
Plan of Correction: The batteries were given to the Supervisor a week prior to the inspection by the Specialist. He will remind the staff where they are and have them put them into the radio and flashlight.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
