Comments:
A monitoring inspection was initiated on 04/28/2022 and concluded on 04/28/2022. There were a total of 38 children with 7 staff members present. Children were observed during snack time, engaged in music and movement and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-A
Description: Based on staff record review all applicants for employment, employees, applicants to serve as volunteers, and volunteers did not undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
Staff #3 had a background check dated 2/13/2017 (expired 2/13/2022). The updated background check was mailed on 4/11/2022.

Plan of Correction: We will send out background checks 2 months prior to them expiring.

Standard #: 8VAC20-780-160-A-1
Description: Based on staff record review, documentation of the TB screening was not submitted at the time of employment.
Evidence:
Staff #1 (date of hire 3/7/2022) had a TB test dated 3/10/2022 during the inspection.
Staff #2 (date of hire 12/3/2021) had a TB test dated 12/6/2021 during the inspection.

Plan of Correction: We will have our staff get their TB screening prior to their first day.
