22.1 Early Childhood Care and Education

Technical Assistance:
The center demonstrated compliance related to the installation of the carbon monoxide detector.

Technical assistance was offered related to: background checks and portability of background checks, staff training for becoming program lead.

Comments:
An unannounced monitoring inspection was conducted from 10:35 am to 1:15 pm. There were 30 children in care of 7 staff members. The children were observed playing outdoors, using puzzles, eating lunch and taking naps. Diapering was also observed. The lunch menu included turkey sausage, green beans, apples, biscuits and milk.
There were no children with medications on site. 4 staff records, 3 children's records and center's policies were reviewed. Emergency drill records were also reviewed. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not obtain results of an out of state central registry finding for an employee that has resided outside of Virginia in the last five years, within 45 days of employment.
Evidence:
1. Staff # 1's first day of employment was 1/13/22.
2. Staff # 1's record indicated that she resided in Kentucky within the last 5 years.
3. A request for the Kentucky central registry was not submitted for Staff # 1 to this date.

Plan of Correction: Ensure all in state and out of state background checks are updated and current as well as central registry.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review and staff interview, the center did not obtain result of a Virginia central registry finding for an employee within 30 days of employment.
Evidence:
1. Staff # 1's first day of employment was 1/13/22.
2. Staff # 3 stated that the central registry form was submitted but the results have not been obtained yet.
3. There was no documentation to show that a follow up related to Staff # 1's request was completed with the agency, by the center.

Plan of Correction: Ensure all in state and out of state background checks are updated and current as well as central registry.

Standard #: 8VAC20-780-210-A
Description: Based on records review and staff interview, the center did not have a program lead qualified staff in each classroom.
Evidence:
1. It was observed that Staff # 1 was observed to be working in the Infant classroom.
2. Staff # 2 was was observed to be working in the 3's classroom.
3. There were no other staff working in these classrooms.
3. Staff # 3 stated that Staff # 1 and Staff # 2 were hired in the capacity to be program lead in the respective rooms.
4. Staff # 1 and Staff # 2's records did not have any documentation to show that they met the requirements to be program lead.

Plan of Correction: All staff is in the process of taking the CDA class to be qualified as Lead teachers

Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interview, areas outside the center were not maintained in a safe condition.
Evidence:
1. The pour in place resilient surfacing on the playground (in front of the school age classroom) was broken at two different spots, posing a trip hazard.
2. Staff # 3 stated that the maintenance department told her in October 2021 that it will be fixed when the weather gets better.

Plan of Correction: I put a work order in to maintenance requesting repairs to playground to fix tripping hazards.

Standard #: 8VAC20-780-500-A
Description: Based on observation, children's hands were not washed were not washed with soap and running water after diapering.
Evidence:
1. In the infant classroom, Staff # 1 changed a child's diaper.
2. Staff # 1 did not wash the child's hands with warm running water and soap after the diaper change.

Plan of Correction: Staff has already been retrained in diaper changing procedure.
