Comments:
The following inspection took place between the hours of 9 am through 10 am. Today, I met with the staff of Kiddie Academy located in Leesburg, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident self-reported by the center to the Office of Child Care Health and Safety (Fairfax).

Information gathered during the inspection determined that there were (2) two non-compliances with the applicable standards. Violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on interview and review of written statements, the center failed to ensure that the center's activities were maintained in compliance with the center's own policies as required by licensing standards.

Evidence -
1. On the date of inspection (04/22/2022) it was determined that on 04/21/2022, between the approximate time of 11:20 am to 11:30 am, Staff #1 and Staff #2 failed to conduct a name-to-face check when leaving the playground and again upon their return to the Busy Bees classroom.
2. The Center Director stated (during an interview on the date of inspection) that a name-to-face check is required (by center policy) during transition; conducted once when leaving an area with a group of children, and again at arrival to the new area. This ensures that all children in attendance are present. These checks are also conducted at regular intervals throughout the day.
3. As a result of not conducting a name-to-face, Child #1 was left unsupervised on the center playground for 19 minutes.
4. This incident was self-reported to the Office of Child Care Health and Safety by the Center Director on 04/21/2022.

Plan of Correction: All staff will be retrained on name-to-face. Administrative action was taken on both Staff #1 and Staff #2, and blind spots will also be addressed on playground.

Standard #: 8VAC20-780-340-F
Description: Based on review, a child under 10 years of age was not always within actual sight and sound supervision of staff.

Evidence -
1. On the date of inspection (04/22/2022), it was determined that on 04/21/2022, Child #1 (4 years of age) was left out on the preschool playground for approximately 19 minutes.
2. The amount of time was determined by a time stamp placed on a video recording of this incident.
3. This incident was directly linked to the failure of Staff #1 and Staff #2 to do a name-to-face for all children directly in their care.
4. After 19 minutes, Child #1 was located on playground by Staff #1 and brought back to room. No other staff or classroom children were outside at that time.

Plan of Correction: All staff will be retrained on name-to-face. Administrative action was taken on both Staff #1 and Staff #2, and blind spots will also be addressed on playground.
