A complaint inspection was initiated on 3/1/2022 regarding behavior guidance, and care and protection of children. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and special care and emergencies. A total of 1 child record and 2 staff records were reviewed. Information gathered during the inspection determined that the complaint was not valid. There were, however, unrelated areas of non-compliance with applicable standards or law and violations that were found and documented on the violation notice issued to the program. Standard #: 22VAC40-185-160-C Complaint related: No Description: Based on a review of staff records, 1 out of 2 staff records did not have a TB screening at least every 2 years from the first initial screening.
Evidence:
1. Staff #1's (doh: 7/6/2020) most recentTB screening on file during the inspection was dated 9/23/2019.Plan of Correction: The staff member received subsequent TB test results on 3/5/2022.
Standard #: 8VAC20-780-140-A Complaint related: No Description: Based on a review of records, all children did not have a physical examination before the child's attendance or within 30 days after the first day of attendance.

1. Child #1 (doe: 7/14/2019) did not have a physical examination on file during the inspection.Plan of Correction: Moving forward, we will ensure that we communicate with parents consistently on what is required for children's records prior to enrollment.
