Comments:
A renewal inspection was initiated on April 21, 2022 and concluded on April 26, 2022. There were twenty-eight children present, ranging in ages from two years to five years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records a three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on observation and review, facility did not ensure there is a written record of children in attendance that documents the arrival and departure of each child as it occurs.

Evidence: At approximately 12:15 PM, twenty-eight children were observed present and twenty-two children were noted on the attendance sheet.

Plan of Correction: Make sure attendance is checked daily.

Standard #: 8VAC20-780-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable manner.

Evidence: In the 3's room, there is peeling paint on the baseboards, lower door frame and window sill.

Plan of Correction: Will repaint door frames.

Standard #: 8VAC20-780-280-B
Description: Based on observation, facility did not ensure hazardous substances are kept locked.

Evidence: Spray Disinfectant labeled "Caution....keep out of reach of children" was stored on the sink counter in the hall women's restroom that is used by children.

Plan of Correction: Have removed disinfectant spray.

Standard #: 8VAC20-780-350-B-3
Description: Based on observation, facility did not ensure there is at least one staff for every eight children that are two years old.

Evidence: At approximately 12:15 PM, one staff was present with eleven children in the 2's classroom. Child #3, age two years, was the youngest in the group.

Plan of Correction: Have a new staff member to join that class.

Standard #: 8VAC20-780-500-B
Description: Based on observation, facility did not ensure all requirements are met in the diapering area.

Evidence: The following was observed:
1. A child's lunch box and a piece of child art was laying on the diapering surface in the play room.
2. The diaper disposal system in the two year old room required staff to push the button to open the lid.

Plan of Correction: Will keep diapering area clear of any objects.

Standard #: 8VAC20-780-540-B
Description: Based on observation, facility did not ensure first aid kits are accessible to staff but not accessible to children.

Evidence: While children were outside playing, the first aid kit was stored on the outside steps used to enter the building. A child was observed picking up the first aid kit. The kit was accessible to children.

Plan of Correction: Will keep first aid kit away from children.
