Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:45am-11:45am. There were a total of 38 children with seven staff members present. Children were observed during circle time, diapering, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on review of documentation, an employee did not have a completed sworn statement or affirmation.
Evidence:
1. Staff #1's record (Date of hire-02/21/2022) contained a signed sworn statement after the first day of employment dated 03/01/2022.

Plan of Correction: Staff will complete a Sworn Statement prior to the first date of employment.

Description: Based on review of documentation, the licensed or registered child day program or family day system does not have a central registry finding within 30 days of employment or volunteer service.
Evidence:
1. Staff #3's record (Date of hire-03/16/2022) does not have a Central Registry Results in the record.
2. Staff #4's record (Date of hire-12/27/2021) does not have a Central Registry Results in the record.

Plan of Correction: A Central Registry Search will be completed for both employees.

Description: Based on observation, electrical outlets did not have protective covers that are of a size that cannot be swallowed by children.
Evidence:
1. In the Toddler room, four electrical outlets were not covered with protective covers.

Plan of Correction: Outlet covers were put on the electrical outlets at the time of the inspection.

Description: Based on review of documentation, documentation was not maintained for evacuation, shelter in place, and lockdown drills.
Evidence:
1. The Evacuation Drill log was missing the following information: notification method, number of staff, weather conditions, and problems encountered, if any.

Plan of Correction: The center will use the department's model form to document evacuation drills.
