Comments:
An unannounced monitoring inspection was conducted today between the hours of 12:00pm-2:00pm. There were a total of 31 children with seven staff members present. Children were observed during lunch time, diapering, handwashing, and nap time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the center did not conduct a five year repeat check for Central Registry results.
Evidence:
1. Staff #3's (Date of rehire-12/13/21) record did not have documentation of a five year repeat Central Registry results. The Central Registry results was dated 04/21/2017.

Plan of Correction: A new Central Registry search will be conducted.

Standard #: 22.1-289.035-B-2
Description: Based on record review, staff did not submit to fingerprinting prior to the first day of employment.
Evidence:
1. Staff #1 (Date of hire-3/23/22) did not have documentation of fingerprint results in the record.
2. Staff #3 (Date of hire-12/13/21) did not have documentation of fingerprint results in the record.

Plan of Correction: Staff will be sent to get fingerprints and the center will send the results to the licensing inspector.

Standard #: 8VAC20-780-140-A
Description: Based on record review, each child did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child #1 (Start date-5/20/19) did not have documentation of a physical examination.

Plan of Correction: The center will ask the parents for a copy of a physical examination and document the record.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's record did not contain a second emergency contact.
2. Child #1's record and Child #3's record did not contain documentation of an annual update.

Plan of Correction: The center will get this information from the parents and document the record.

Standard #: 8VAC20-780-500-B
Description: Based on observation, it was determined that the facility did not ensure that a nonabsorbent surface for diapering or changing is used.
Evidence:
1. In the Toddler classroom, the changing pad had three one inch tears on the surface.

Plan of Correction: The center will purchase a new diaper changing pad for the classroom.

Description: Based on record review and interview, the center did not implement a monthly practice evacuation drill.
Evidence:
1. The evacuation drill log did not have documentation of a monthly evacuation drill for the months of January, March, and April.
2. Staff #4 confirmed that a practice evacuation drill was not conducted for the months of January, March, and April.

Plan of Correction: The center will practice additional monthly drills to make up for the missed drills.

Standard #: 8VAC20-780-560-M
Description: Based on observation, a child was allowed to drink or eat while walking around.
Evidence:
1.In the Toddler classroom, a child was allowed to walk around while drinking a bottle.

Plan of Correction: Children will not be allowed to walk around while eating and drinking.

Standard #: 8VAC20-780-570-A
Description: Based on observation, children using infant seats or high chairs shall be supervised during snacks and meals.
Evidence:
1. In the Infant 2 classroom, two older infants were seated at a table eating while unsupervised. One older infant was seated in a highchair while eating unsupervised.

Plan of Correction: The children will be supervised during meals and snacks.

Standard #: 8VAC20-780-570-E
Description: Based on observation, a prepared infant bottle was not dated.
Evidence:
1. Two bottles located in the refrigerator in the Infant 2 room were not dated.

Plan of Correction: Bottles will be labeled with the child's name and dated.
