Comments:
Today an unannounced Monitoring Inspection took place between the hours of 12:20pm and 1:40pm. There were 5 classrooms observed with a total of 68 children with 16 staff within the supervision guidelines. The children were observed eating lunch, being fed bottles, hand washing, preparing for their rest time and other organized activities. A complete inspection of the physical plant, 6 children and 3 staff records, evacuation drills, 2 emergency medications and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, 3 out of 3 staff did not submit their tuberculosis screening at the time of employment.

Evidence:
1. Staff #1 (doh: 2/14/2022) had a TB test with a date of 2/17/2022.
2. Staff #2 (doh: 1/6/2022) had a TB test with a date of 1/28/2022.
3. STaff #3 (doh: 12/7/2021) had a TB test with a date of 1/26/2022.

Plan of Correction: We will make sure we have the TB screenings at the time of employment moving forward.
