Comments:
An unannounced monitoring inspection was conducted on 05/02/2022 from 10:05 am - 2:10 pm. At the time of entrance there were 117 children in care with 18 staff supervising. The children in care ranged in age from 6 months - 5 years. Children were observed during morning program time, outdoor play and nap. Records were reviewed for five staff and five children.

Violations were found in the areas of administration, special care provisions and background checks. These violations are listed on the violation notice and were reviewed with administrative staff at the conclusion of the inspection.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1, hire date 08/16/2018, lacks the results of a criminal record check, sex offender check and central registry check from the State of California.
a. Staff 1 indicated on her sworn statement she had resided in California within the past five years.
b. Administrative staff confirmed that background checks from the State of California were not on file for staff 1.

Plan of Correction: *California Criminal Background Check - contacted, waiting on response - 5/6/22
*Sex Offender Background Check was submitted - 5/4/22
*Central Registry was submitted - 5/6/22
*New Director will verify that employees hired before 2020 are in compliance with background check policy for out of state checks.

Standard #: 8VAC20-780-160-C
Description: Based on record review, the center failed to ensure at least every two years from the date of the first initial screening or testing staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The most recent TB screening on file for staff 1 was conducted on 02/17/2020.

Plan of Correction: Employee completed TB test.

Description: Based on review, the center failed to ensure that the written list of children's allergies and sensitivities was kept confidential in each room or area where children are present.

Evidence:
The allergy list was posted on the information board in each classroom without any type of covering to ensure the information was confidential.

Plan of Correction: Cover sheet was prepared and placed over allergy cards in the classrooms.

Standard #: 8VAC20-780-520-C
Description: Based on review, the center failed to ensure if diaper ointment or cream is used, the following requirements shall be met:
a. These products shall be labeled with the child's name;

Evidence:
1. Two diaper ointments, in the toddler classroom, were not labeled with the child's name.
a. These products were stored inside of a labeled plastic bag.

Plan of Correction: Diaper cream was labeled and staff reminded that products must be labeled in the labeled bag.
