Technical Assistance:
The licensee's license to operate a CDC is expiring 5/21/2022. A renewal application has not yet been received. The program director was encouraged to submit a complete application to the Fairfax licensing office as soon as possible.

Comments:
Today an unannounced Renewal Inspection took place between the hours of 10:15am and 11:30am. There was 1 classrooms observed with a total of 4 children with 3 staff within the supervision guidelines. The children were observed participating in circle time activities, singing and being read to in Spanish and other organized activities. Hand washing procedures were observed. There were not medication on site today. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today's inspection and can be viewed on the violation notice.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review, staff members have not submitted subsequent tuberculosis (TB) screenings every two years from the date of the initial screening.
Evidence:
1. Staff #1's most recent TB screening is dated 12/30/2018.
2. Staff #3's most recent TB screening is dated 11/16/2018.

Plan of Correction: Staff #1 provided TB test taken on 2/12/2022. Staff #3 will provide the test on 5/3/2022

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interview, a written care plan has not been obtained for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: Child #5's physical exam documents that the child has a diagnosed food allergy. The program director confirmed that they do not have a written care plan for Child #5.

Plan of Correction: Parents have already been asked. They should provide it ASAP.

Standard #: 8VAC20-780-245-A
Description: Based on record review, staff have not completed a minimum of 16 hours of training annually.
Evidence:
1. Staff #2's record does not contain documentation of any training completed in 2021.
2. Staff #4's record contains documentation of 2.5 hours of training completed in 2021.

Plan of Correction: All staff will make sure to complete training of 16 hours every year.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food containers were not clearly dated and labeled in a way that identifies the owner.
Evidence: Three lunch boxes were observed sitting in chairs. Three out of three lunch boxes were not labeled with a child's name or today's date.

Plan of Correction: Parents were asked to provide labels with names and dates. Already corrected.
