An unannounced monitoring inspection was conducted today. There were 15 children present with two staff. The children were finishing lunch and resting. Physical plant, staffing and supervision, special care and emergencies, nutrition, two children's files and two staff files were reviewed.
Violations were noted on the violation notice.

Standard #: 8VAC20-780-160-C Description: Based on a review of staff files, the facility failed to ensure that staff have repeat TB tests or screenings every two years.
Evidence:
1. There are no current TB test or screening results on file for staff #2. The document on file is dated 10-15-18.Plan of Correction: The program manager reported that staff #2 has scheduled an appointment for a TB test or screening.
