Technical Assistance:
Discussed the procedures of properly sanitizing tables used for snack.

Comments:
An unannounced monitoring inspection was conducted today. A total of 26 children with 3 staff were present. Observation of the children arriving at the program was made. The children proceeded to line up to wash hands prior to snack time. Snack today was milk with yogurt. The children sat in groups of 3 and either completed homework or logged onto their laptops. Some children opted to draw and color on coloring sheets provided by staff. All medication was reviewed, 4 children's records were reviewed and staff's certifications were verified. Background checks and annual training and staff qualifications are reviewed annually at the main office. This inspection occurred approximately between 1:50pm and 3:20pm. Description: Based on review of staff records, one record was incomplete.
Evidence:
Staff #3's documentation does not include an emergency contact.

Plan of Correction: We will speak with the main office about maintaining all substitutes emergency contacts electronically.

Standard #: 8VAC20-780-550-G
Description: Based on review of emergency evacuation drills, documentation was incomplete for one drill.
Evidence:
The December evacuation drill did not include: Identity of the person conducting the drill;
1. The date and time of the drill;
2. The method used for notification of the drill;
3. The number of staff participating;
4. The number of children participating;
5. Any special conditions simulated;
6. The time it took to complete the drill;
7. Problems encountered, if any; and
8. Weather conditions.

Plan of Correction: The site supervisor is aware of this documentation and will ensure that all future drills will be documented entirely.

Standard #: 8VAC20-780-550-P
Description: Based on review of the center's record of injuries, a complete record is not maintained.
Evidence:
The record of injuries are only maintained at the center for the current month. Each month, the reports are sent to the main office.

Plan of Correction: We will ensure that a copy of each report is maintained at the center.
