Technical Assistance:
Discussed the importance of following the center's own policies and procedures and the need to re-train staff as needed.

Discussed the portability of background checks conducted in Virginia during the past five years for new hires.

Comments:
A focused inspection was conducted this date to evaluate the corrective actions provided from a previous licensing inspection. A total of 43 children with 10 staff were onsite today. Walk thru and observation of the children was completed in the Infant, Beginners, PreK 1 and 2 classrooms. Staff interviews conducted and recordation was reviewed. Three new staff hired since the last inspection and all three files were reviewed. A review of documented class observations and additional trainings was completed. Some violations were found today unrelated to the areas found out of compliance during the August 2021 inspection. A corrective action was provided onsite. Standard #: 8VAC20-770-60-B
Description: Based on review of staff records, one background check was not obtained prior to hire.
Evidence:
Staff #2 was hired on 05/09/2022. A Sworn Statement was not completed and/or maintained.

Plan of Correction: We will have the staff completed a Sworn Statement and will forward it to the Licensing Inspector for review.

Standard #: 8VAC20-780-40-E
Description: Based on staff interviews, a review of the center's own video recording and a review of the electronic system used by staff, one staff failed to comply with the center's own policy.
Evidence:
1. The center's own procedures for identifying where attending children are at all times requires staff to enter the location of each child at every transition on an electronic device.
2. The center's own video recording of the morning of 05/10/2022 recorded Staff #3 failing to enter the face to name transitions for the Beginner's class at two thresholds leading to the outside playground.
3. Staff #3 was unable to navigate the electronic system and stated they were unable to locate the transitions for the morning of 05/10/2022.

Plan of Correction: This staff will be retrained on the use of the Tadpole system and informed of the importance of entering the location of every child.

Standard #: 8VAC20-780-70
Description: Based on review of staff records, 2 records were incomplete.
Evidence:
1. Staff #2's record did not contain an emergency contact.
2. Staff #3's record did not contain 2 references that are required prior to hire.

Plan of Correction: We will obtain the information from Staff #2. Going forward we will ensure references are received for any new hires or re-hires before their first day of employment.

Standard #: 8VAC20-780-450-A
Description: Based on observation of the Beginner's Classroom, linens were not found for one cot.
Evidence:
One cot occupied by one child did not have a bottom cover.

Plan of Correction: We will request the parents to provide a bottom cover. Until a bottom cover is received, one will be provided by the center.
