Comments:
An unannounced monitoring inspection was conducted today from 9:45am-11:30am. There were 13 children directly supervised by 2 staff and the classroom was in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 5 children?s records, 2 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Parent drop off procedures were observed. Child were observed participating in morning check-in and circle time, reciting the alphabet, counting up to 50, and reciting the pledge of allegiance. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.058
Description: Based on staff interview, the center has not ensured that each building that was built before 2015 and that houses a child day program that serves preschool-age children is equipped with at least one carbon monoxide detector.

Evidence: There was no carbon monoxide detector installed at the center.

Plan of Correction: Carbon monoxide detector to be placed on each floor.

Standard #: 8VAC20-780-40-M
Description: Based on observation and staff interview, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions.

Evidence: Child #2 has a food allergy and has emergency medications on site. There was not a written list of children's allergies in the classroom.

Plan of Correction: Current list of students allergies, sensitivities, and dietary restrictions will be maintained in the classroom.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written allergy care plan for each child with a food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: Child #2 has a food allergy and has emergency medications on site. Child #2 does not have an allergy care plan signed by a physician on file.

Plan of Correction: Allergy care plan with physician instructions will be maintained for each student with listed allergy.

Standard #: 8VAC20-780-510-E
Description: Based on medication review, the center did not ensure that long term medications are accompanied by a signed physician authorization.

Evidence: Child #2 had two emergency medications on site and did not have a physician authorization on file.

Plan of Correction: Medications return to parents of Child #2.
We will only accept medications with proper physician authorization forms.
