Technical Assistance:
Discussed lead teacher qualifications and requirements of what to have in the file for proof of qualification.

Comments:
An unannounced monitoring inspection was conducted on 5/12/2022 from 9:22am to 11:10am. At the time of entrance, 45 children were in care with 11 staff members present. Children were observed doing the following: making gifts for their parents, playing outside, playing with toys, painting, art activities, working on puzzles, playing with sand and water tables and building with wood blocks. The site was organized and contained developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills and attendance records were reviewed. There was not medication on site. Description: Based on interview, the center did not develop written procedures for injury prevention.
Evidence:
1. Staff #3 stated they did not have written procedures for injury prevention.

Plan of Correction: I will create one. We review playground and facility safety regularly at staff meetings, but will create written plan and review regularly,.

Standard #: 8VAC20-780-40-N
Description: Based on interview, the center did not develop written playground safety procedures.
Evidence:
1. Staff #3 stated they did not have written playground safety procedures.

Plan of Correction: I will create one. We have playground rules posted on our gate and review regularly with teachers. Will review playground safety each month at our staff meetings.

Standard #: 8VAC20-780-50-A
Description: Based on observation, staff and children's records were not treated confidentially.
Evidence:
1. During the inspection, the supplemental document for the inspection dated 2/17/2016 was posted on the bulletin board. This document is considered confidential.

Plan of Correction: This was removed immediately during the inspection. Was inadvertently posted-now removed-procedures for posting reviewed.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment of the center outside was not maintained in a safe and operable condition.
Evidence:
1. A table filled with pea gravel had a large crack approximately 4 inches long on the side of it.

Plan of Correction: We will throw out the playground table. Table edge cracked and we have already thrown it out.

Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the 2 1/2 year old classroom, a container of clorox wipes and lysol spray were located in an unlocked cabinet above the sink.
2. In the 3 year old classroom, a container of clorox wipes and lysol were in an unlocked bathroom cabinet.
3. In the 3 and 4 year old classroom, a lysol spray and sanitizer was in an unlocked bathroom cabinet.
4. In the hallway bathroom for girls, a glade spray was located on a shelf above the changing table.

Plan of Correction: We will lock all the cleaning products in the cabinets and remove the glade. Teachers were reminded by email right after inspection to lock all cabinets. Glad spray was removed from cabinets locked. Children are supervised at all times and cleaners stored in upper cabinets only.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation, the findings of the most recent inspection of the facility was not posted.
Evidence:
1. The inspections for 9/24/2019 and 2/19/2019 was posted on the bulletin board, but the inspection dated 1/29/2021 was not posted.

Plan of Correction: I will post in the future. During the Pandemic no visitors/family allowed in our facility, so we created a virtual bulletin board on our website. elcvienna.org/preschool under resources.
