Comments:
An unannounced monitoring inspection was conducted on 04/14/2022, from 12:00pm - 2:30pm and a follow up was conducted on 04/25/2022 and 4/29/2022 in response to a self-reported incident that was received by the licensing office on 03/28/2022, regarding behavioral guidance and forbidden actions. . There were 85 children (infants-5yrs.) directly supervised by 14 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play and eating lunch. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on staff interviews, records reviewed, and observations made, the evidence gathered during this investigation does not support violations; however, violations not related to the self-reported incident, were identified during the course of the inspection and can be found on the violation notice. Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child #5's record contains documentation of immunizations that was last updated on 9/10/2020.

Plan of Correction: Parents were emailed to obtain updated documentation.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
Child #1 and Child #2 were both listed as having diagnosed food allergies, and both had emergency medication on-site; however, there was no allergy care plan on file for these children.

Plan of Correction: Written care plan emailed to parents.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant formula was refrigerated, but not dated.

Evidence:
Two bottles containing infant formula were observed in the refrigerator labeled with the child's name, but without a date.

Plan of Correction: Fixed while inspection was in progress.
