Comments:
An unannounced monitoring inspection was conducted with the assistant director and director on May 9, 2022 beginning at 11:00 AM and ending at 1:04 PM. There were 52 children in the care of 9 staff members. The children were observed playing outside on the playground. Children were also observed eating lunch which consisted of hamburgers, potato chunks, apple sauce, and milk. Five children?s records were reviewed. Five staff records were reviewed and in compliance. The medications were reviewed and in compliance. The most recent emergency escape drill was documented as 4/27/22. The most recent shelter-in-place drill was documented as 4/5/21. The most recent lockdown drill was documented as 5/4/22. The most recent health inspection was dated 1/31/2022. The most recent fire inspection was dated 9/15/21.

Standard #: 8VAC20-780-130-A
Description: Based on a review of documentation, not all children had documentation of immunizations in their record prior to their first day of attendance at the center. Evidence: Child A's record was missing documentation of immunization.

Plan of Correction: The director sent an email to the parents requesting the missing documentation during the inspection.

Standard #: 8VAC20-780-140-A
Description: Based on a review of documentation, not all children had documentation of a physical examination in their record within 30 days of their first day of attendance at the center. Evidence: Child A was missing documentation of of physical examination.

Plan of Correction: The director sent an email to the parent requesting the missing documentation during the inspection.
