Comments:
A complaint inspection was conducted on April 7, 2022 and concluded on April 14, 2022. A complaint was received by the department regarding allegations in the area of inappropriate behavioral guidance and staff qualifications. Based on observation and review of documentation, the complaint was determined to be valid.
The evidence gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Complaint related: Yes
Description: Based on review of two staff files and staff interview, the facility failed to have a program leader in each grouping of children that meets the qualifications of a lead teacher. Evidence: Staff B does not have documentation to show she meets the qualifications to be a lead teacher.

Complaint related: Yes
Description: Based on observation, the center failed to ensure that children were not placed in any space that a child can not freely exit himself, such as a high chair when not used for its intended purpose. Evidence:
1. On April 7, 2022, Staff A told Child A (age 2) "if you don't start listening, I'm going to put you in the high chair for time out. Do you want to sit in the high chair?" 2. The parents of Child B (age 2) reported the following: Staff C said that "the child would be screaming and crying and throwing a tantrum so bad that she would be rocking the high chair back and forth to the point that she could have fallen over". During these times, they did not remove Child B from the high chair, but instead stabilized it by holding it down so she couldn?t shake it.
