Reminder to make sure all staff are aware of the children withallergies and the location of the written allergy list with the date the list was updated.

Discussed playground resilent surfacing. A new playground equipment was installed waiting for the resilent surfacing to be delivered.

Comments:
An unannounced monitoring inspection was conducted on May 04, 2022. At the time of entrance there were 43children and 7 staff members present with children. The sample size consisted of 3 children's and 6 staff records. Children were observed participating in coloring activity, circle time and freely playing. Areas of non-compliance are identified on the violation notice. The results of the inspection and the violations cited were reviewed and verified with the center director.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the licensee did not ensure a written care plan for each child with diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
Child #2 did not have allergy action plan provided with the record. Physical confirmed the allergy was doctor diagnosed.

Standard #: 8VAC20-780-260-A
Description: Based on record review and staff interview, the licensee did not ensure annual fire inspection was completed annually.

Evidence: The last fire inspection was documented was December 2020 . Center director confirmed the annual fire inspection was not completed as of May 4, 2022.

Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interview, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: In the facility, there were multiple area in each classroom with peeling paint. Staff confirmed they were aware of the peeling paint in the building. The boys restroom in the open classroom ( PRE-K) there is a toilet that will not flush. In the Early 2 year old classroom, the door into the bathroom, had a protrude screw and the door will not shut properly. Confirmed with the staff the concern with the door.

Description: Based on observation, the facility did not ensure when a child is placed in an infant seat or high chair, the protective belt was fastened securely.

Evidence: 1. Five toddlers were observed sitting in bucket seats in the feeding/activitiy table. 2. The safety belts in the five occupied bucket seats were not fastened.
