An unannounced monitoring inspection was conducted today.
There were 15 children present with a teacher and a substitute.
The children were playing outdoors.
Two child files and two staff files, the physical plant, staffing supervision and emergency drills were reviewed.

Evidence:
1. The sworn statement on file for staff #1 does not ask for previous states of residence for the last five years.Plan of Correction: The program manager will ensure that all background checks are completed on the correct forms.
