Licensing inspector discussed new regulations, emergency preparedness plan and DOE templates with the owner.
- Comments:

A monitoring inspection was conducted on 04/28/2022 from approximately 11:20am-12:50pm. There were 8 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-50-A Description: Based on observation, children and staff records were not treated confidentially.
Evidence.
Children and Staff records were located on a shelf in the corner of the Butterflies classroom.Plan of Correction: Documents were moved to the office.
Standard #: 8VAC20-780-60-A Description: Based on record review, the record of each child was not complete.
Evidence:
Child A was missing the employer, the employer number and employer address of both parents.
Child B did not have documentation of proof of identification.Plan of Correction: Parent will update the form.

Evidence:
1. Staff A,B and C were missing an emergency contact.
2. Staff A and B were missing proof of identification and proof of orientation.
3. Staff C was missing 2 required references, proof of orientation and job title.Plan of Correction: All missing documents will be added.
Standard #: 8VAC20-780-245-L Description: Based on interview, there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence:
When asked which staff obtained daily health observation training, Staff D stated no one.Plan of Correction: We are planning to register the training course.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place.
Evidence:
Hazardous cleaning liquid was unlocked in a bucket in the Caterpillar 1 and Caterpillar 2 rooms.Plan of Correction: We will buy a small storage bin with a lock.
Standard #: 8VAC20-780-540-C Description: Based on observation, the first aid kit did not have all of the required components.
Evidence:
The first Aid kit was missing scissors and activated charcoal.Plan of Correction: We will add the scissors and buy the charcoal.
