Technical Assistance:
There shall be a flexible daily schedule for infants based on their individual needs.
For toddlers and preschoolers there shall be a posted daily schedule that allows for flexibility as children's needs require.

Comments:
A monitoring inspection was initiated on 5/16/22 and concluded on 5/17/22. There were 49 children present, ranging in ages from infant to five, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record review and interview, the center failed to obtain a central registry finding within 30 days of employment.

Evidence:

Staff 2's date of hire is documented as 1/19/22 and orientation was dates 1/19/22. The findings of the central registry background check is dated 4/29/22. The notary was not signed until 2/4/22.

Plan of Correction: In the future newly hired staff will be required to complete the paperwork for central registry background checks upon hire and the documents will be mailed within seven days of signature. If the findings are not received within 30 days of employment the director will follow up. The staff file will be documented with the dates and actions of each step of the process.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:

1. The date of birth for child 1 is documented as 3/11/21. The only immunization record in the file was dated 8/18/21.
2. The date of birth for child 2 is documented as 3/6/20. The only immunization record in the file was dated 3/3/21.

Plan of Correction: The updated immunizations were obtained on 5/17/22 for both children. In the future updated immunizations will be obtained every six months until age two as required by standards.

Standard #: 8VAC20-780-80-A
Description: Based on record review and interview, the center failed to keep a written record of daily attendance that documents the arrival and departure of each child in care as it occurs for each group of children.

Evidence:

1. The attendance log in each classroom did not contain the departure of children each day.
2. The attendance in the 2's classroom had not been taken as of 10:15 AM. Six children were present at that time.

Plan of Correction: A new attendance sheet has been provided to each classroom that provides a space for staff to include when children leave for the day. All staff have been reminded to document children's attendance as they arrive and departure as they leave.
