Comments:
A monitoring inspection was initiated on 05/09/2022and concluded on 05/09/2022. There were 77 children present and 17 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. An in-person tour of the center was also conducted. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The center's poured-in-place resilient surfacing had cracks and dents in the surface area in the fall zones of the outside play structures.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
A work order has been placed to repair the surfacing.

Standard #: 22VAC40-185-440-H
Description: Based on observation the center did not ensure all requirements were met for cribs.
Evidence:
A child was observed sleeping in a crib that had more than 1 inch of space between the mattress and crib.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The sheets the center ordered were too small causing the sides of the mattress to curl. New sheets will be ordered and tested to ensure they fir snug and the mattress does not have more than 1 inch of space between the mattress and the crib.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. In the toddler room the door at the bottom of the changing table was broken and the cabinet above the table was chipped with particle board exposed.
2. There was rust on the metal portions of some of the tricycles on the playground.
3. The large red pipe had peeling paint and was in reach of children.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
Work orders have been placed to repair physical plant issues.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place.
Evidence:
1. The bathroom in the front hallway had 3 bottles of disinfectant spray on a shelf above the toilets.
2. In the pre-k rom there was a bottle of disinfectant spray in an unlocked cabinet.

Plan of Correction: The following corrective action was provided by the director during the interview.
Evidence:
All chemicals were locked during the inspection. The director will remind staff to lock all hazardous substances.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
A staff member in the twos room was observed changing a child's diaper and did not wash her hands or the child's hands after the diaper change.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
Staff will be retrained in diapering and handwashing procedures and monitored for future compliance.
