Comments:
A renewal inspection was initiated on 5/18/2022 and concluded on 5/18/2022. There were a total of 48 children with 6 staff members present. Children were observed engaged in individual activities, number line, fine motor and butterfly art. Medications and center policies were reviewed. The playground and surfacing was inspected. A total of 2 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records and owner interview, management has not at least every two years from the date of the first initial screening obtained and submit the results of a follow-up tuberculosis screening.
Evidence: Staff #2 has not obtained an updated TB screening. The TB screening on file was dated 2/5/2020. The owner stated that the staff has not updated the TB screening.

Plan of Correction: The staff will get her updated TB screening.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records and owner interview, there was no documentation that two or more references were checked before employment.
Evidence: Staff # 1, date of hire 5/9/2022 does not have documentation of 2 references on file. The owner stated the staff was referred by parents at the child care but he has not documented the reference check.

Plan of Correction: Owner will call references and document the results in the record.
