Comments:
A renewal inspection was initiated on April 20, 2022 and concluded on April 20, 2022. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
1. The record for staff #5 (date of last TB screening 2/15/19) contained an updated TB screening on 4/07/21.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. In classroom #4, an electrical outlet on the wall had drywall missing from around two of the sides of the outlet cover exposing the back of the outlet.
2. The fence surrounding the playground had a board to cover gap between the end of the fence and the building. The board was not attached at the top of the fence and the building allowing the board to be moved. A four to five foot drop is located behind the fence.
3. In bathroom #1, peeling paint was observed exposing drywall.

Description: Based on observation and an interview, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #3, a container of Lysol wipes labeled "Danger," "Hazards to Humans" and "Keep Out of Reach of Children" and a container of Clorox labeled "Harmful" and "Hazards to Humans? in a unlocked bottom cabinet of a file cabinet.
2. In vehicle #1 used for transportation, a container of Lysol wipes labeled "Danger," "Hazards to Humans" and "Keep Out of Reach of Children" and in vehicle #2 used for transportation, a container of Lysol wipes labeled "Danger," "Hazards to humans" and "Keep Out of Reach of Children" and a bottle of Windex labeled "Keep Out of Reach of Children" and ?Caution?.

Description: Based on observation and interview, the center failed to ensure that during the day, infants shall be provided with equipment and toys that support large and small motor development.

Evidence:
In classroom #1, infants were observed in care without toys that support large and small motor development. Equipment observed included, highchairs, cribs, two bouncy seats, seven plastic storage containers with drawers for children?s belongings, and a tv.

Standard #: 8VAC20-780-500-A
Description: Based on observation, staff and children's hands were not washed with liquid soap and warm running water and/or disposable wipes before food was served.

Evidence:
Staff was serving food without washing their hands to children sitting at the table who did not wash their hands.
