03/18/22; 04/13/22; 04/21/22 Unannounced inspections were made on these dates in response to information called in to the licensing office in regard to discipline of a child.
Staff and witnesses were interviewed and video tape was reviewed.
Based on evidence gathered during the investigation, there is a preponderance of evidence to substantiate the allegation.
A violation notice was issued.

Standard #: 22VAC40-185-410-1 Description: Based on a review of video recording and interviews with witnesses, the facility failed to ensure that roughly handling a child is forbidden.
Evidence:
1. Two staff reported that on 3/15/22 during lunch, staff #1 was talking to child #1. Staff #1 was verbally correcting child #1, who is two years old, for reaching into another child's plate. The child closed their eyes. Staff #1 roughly handled child #1 by placing her hands on the child's face in an attempt to pry their eyes open to make the child listen to the staff member.Plan of Correction: The staff member no longer is employed with this facility.
The program director conducted an all staff meeting on positive guidance.
