Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training. Center also stated that 3 of 8 new staff files were still waiting the CPS finding from OBI. Center continues to follow-up and document all contact with OBI.

Discussion that the facility has at least (1) one carbon monoxide detector installed on-site.

Comments:
Both a Monitoring Inspection and a Subsidy Health and Safety Inspection (SHSI) were initiated by licensing staff and concluded on 05/19/2022, between the hours of 1 pm to 3 pm. There were 53 children present, ranging from infant to preschool-age, with 12 staff supervising across 6 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; nutrition, and all applicable subsidy regulations. A total of 5 children records, and 8 new staff records were reviewed.

Information gathered during the inspection determined that there was (1) one non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A-2
Description: Based on review, 2 of 8 new staff files indicated that TB documentation was not completed within the last 30 calendar days of the date of employment (start date).

Evidence -
1. On the date of inspection (05/19/2022), the following TB documentation was reviewed: Staff # 2 (start date - 12/13/2021) TB document dated - 06/08/2021; and for Staff #8 (start date - 02/07/2022) TB document dated - 08/20/2021.
2. Both set of documents were outside this 30 day window from their listed start dates.

Plan of Correction: Updated TB documentation will be obtained for both staff and placed on file.
