Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks, staff records, required training, Program Leader qualifications, supervision, emergency drills, transportation, attendance, CPR/First aid certification, carbon monoxide detectors, diaper changing, and allergies.

Comments:
An unannounced monitoring inspection was conducted on 4/28/22 from 9:15am - 12:00pm. During the inspection there were 19 children ages five months old through five years old in care with 10 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating lunch. Records were reviewed for five children while at the center and five staff. Medication, emergency procedures, emergency supplies and transportation procedures were also reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on a review of ten staff records, it was determined that the licensee did not ensure that they obtain a copy of the results of a criminal record check from any state in which the individual has resided in the preceding five years.

Evidence:
1. The record for staff #7 did not contain documentation of a completed criminal record check from Pennsylvania. The sworn statement indicated staff #7 had previously lived in Pennsylvania within the last 5 years.
2. Staff #11 (Program Director) confirmed the sex offender registry check and search of the central registry from the State of Pennsylvania.

Plan of Correction: The facility responded: Staff #7 will complete a criminal record check for Pennsylvania. All new staff who have resided outside the state of Virginia will complete all the required background checks form that state.

Standard #: 8VAC20-780-520-B
Description: Based on observation and interview, it was determined that the facility did not ensure that sunscreen shall be inaccessible to children under five years of age.

Evidence:
1. The Licensing Inspector observed a can of sunscreen in a cubby in the Preschool classroom.
2. Staff #11 (Program Director) confirmed that the sunscreen was accessible to the children present in the classroom.

Plan of Correction: The facility responded: Corrected during the inspection by placing the sunscreen in an upper cabinet that was not accessible to the children. All sunscreen will be stored in a location inaccessible to the children present.

Standard #: 8VAC20-780-570-C
Description: Based on a review of documentation and interviews, it was determined that the licensee did not ensure that the record of each child shall contain the child's feeding schedule.

Evidence:
1. There was no document that contained the feeding schedule for each child who is on
formula.
2. Staff #11 (Program Director) confirmed that there was not a feeding schedule for each child
who is on formula available for viewing during the inspection.

Plan of Correction: The facility responded: We will
develop a list for the infants in care that
lists their feeding schedule and the
brand formula they use.
