A monitoring inspection was conducted on 0513/2022. There were 32 children present and 9 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. In addition to the record review an in-person component was also conducted. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program

Standard #: 22VAC40-185-330-B Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
There were 2 Little Tykes slides in the fenced area designated as the outdoor play area. There was no resilient surfacing in the fall zones of these two slides. The play equipment was over concrete and rubber mats.Plan of Correction: The two slides were removed form the outside play area.
Standard #: 22VAC40-185-340-F Description: Based on observation the center did not ensure all requirements were met when supervising children.
Evidence:
1. A staff member in the first classroom on the left was observed leaving the classroom with 6 three-four year-old children, unsupervised, while retrieving supplies in another area of the building.
2. A staff member in the first classroom on the right was observed leaving the classroom with7 three-four year-old children, unsupervised, while retrieving supplies in another area of the building.Plan of Correction: The following corrective action was provided by the Director during the exit interview: The Director will retrain staff on supervision requirements and discuss a system to get supplies when needed without leaving children in the classrooms by themselves.
Standard #: 22VAC40-185-500-A Description: Based on observation the center did not ensure all requirements were met when handwashing.
Evidence:
A staff member was observed wearing gloves while assisting a child in the bathroom and changing a pull-up. The staff member did not remove the gloves or wash her hands before helping the next child in the bathroom.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will retrain staff on proper handwashing during diapering and helping children using the bathroom.
