Technical Assistance:
Technical Assistance was discussed with the Director regarding:
-updated Sworn Disclosure form
-timing of drills
-child records
-Preservice training

An unannounced monitoring inspection was conducted on 5/17/22 from 9:30am to 11:30am with the Director. There were 42 children in care, ranging in age from two-years to six-years-old, supervised by eight staff. The children were observed eating snack brought from home, transitioning to the playground, and coloring ?Rainbow Fish?. Four child records and three staff records were reviewed. All of the center staff have current certification in CPR and First Aid, as well as Daily Health Observation Training. Three emergency medications and authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 1/22/22 and the most recent Health Inspection on file was dated 11/22/21. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 5/24/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Description: Based on review of three staff records, the center did not obtain documentation of the results of a national, fingerprint-based background check prior to hire for one staff. Evidence: The fingerprints on record for Staff #2, with a hire date of 12/14/21, were dated 1/6/22.

Plan of Correction: Educate all staff in the hiring process. This must be complete before hire.
