Comments:
An unannounced complaint inspection was conducted on site on 5/5/2022. The inspectors arrived at 8:30 AM and departed at 10:00 A.M. There were 21 children 4 staff were present. Reviewed three children records and four staff records during the complaint inspection. The preponderance of evidence of physical abuse was not supported and complaint is not valid.

Description: Based on review of eight staff?s records, the center failed to ensure that all Sworn Statements were completed prior to hire.

Evidence:
1. Staff # 4 started on 3/18/22 on the day of the inspection the Sworn Statement was not completed.

Plan of Correction: All staff will complete the Sworn Statement ASAP.

Description: Based on review of four staff?s records, the center failed to ensure that all staff had a returned CPS background check within 30 days of hire.

Evidence:
1. Staff # 3 with hire date of 2/15/2022 did not have a returned CPS check on the day of the inspection.
2. Staff # 4 with hire date of 3/18/2022 did not have a returned CPS check on the day of the inspection.

Plan of Correction: All staff will complete the CPS ASAP. The new program director will ensure that all records are complete. Follow up will happen within the 30 days of hire and will be documented.

Description: Based on review of three children?s records the center failed to ensure that all records were complete per the standard.

Evidence:
1. Child # 1 did not have allergies and physician?s name and phone number completed on the enrollment form by the parent or the guardian.
2. Child # 2 was missing the start date that the child entered the program.
3. Child # 3 did not have chronic physical problems completed on the enrollment form.

Plan of Correction: All records will be monitored and will be completed by the parent ASAP.

Description: Based on review of four staff?s records, the center failed to ensure that all required item per the standard.

Evidence:
1. Staff # 4 did not have a documented job title, emergency contact with address.
2. Staff # 3 and 4 did not have the two required references.

Plan of Correction: All staff?s records will be reviewed and will had all missing items added to the record. In the future all new employees will have a complete record prior to starting.

Description: Based on observation of the main floor, the center failed to ensure that when all children are in ongoing mixed age grouping, the staff to children ratio is applicable to the youngest child in the group shall apply to the entire group.

Evidence:
1. Upon arrival at 8:30 A.M. Into the center the Inspectors observed 15 children in a mixed ongoing grouping of children. There were 15 children ages 19 months -4 years with staff # 2 and 3. At 8:55 A.M. there were 17 children in care. At 8:58 A.M. the third staff arrives. The required ratio is one staff per five children. This grouping of children required four staff.

Plan of Correction: A staff person left to take a child to school. In the future a staff person will not leave until additional staff are present.
