Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on review of a child's file, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #3's file listed the child had a food allergy, but the center did not have a written care plan for the child to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: I will talk to the mom today to get a plan or change to a preference if it is not a diagnosed food allergy.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside were not maintained in a safe and operable condition.
Evidence:
1. In the Penguin classroom, a soft blue step had 4 corners that were coming unsewn.
2. In the Astronauts classroom, a rug in the reading area was unraveling near the red box.
3. On the playground, 6 tricycles had rust on the frames.
4. On the basketball court, 2 squares tiles had small broken corners.

Plan of Correction: We will remove it today.
We will repair it by Friday.
We will have it repaired and send repair list by Friday.
We will replace the squares or order them by Friday.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to clearly date and label food containers when food is brought from home.
Evidence:
1. Child A had 2 food items from home in the Penguin classroom refrigerator, but they were not labeled with the child's name and date.

Plan of Correction: We will remind staff to label and date food items if parents forget.
