Comments:
A monitoring inspection was initiated on Wednesday, May 18, 2022 and concluded on Tuesday, May 24, 2022.There were ___ children present, ranging in ages from ___ to ___, with ___ staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of___ child records and ___ staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and photograph, the center did not ensure hazardous substances are kept in a locking method that prevent access by children.

Evidence: During the inspection on May 18, 2022, the inspector observed the following in the center where hazardous substances with the labels "keep out of reach of children" "warning" and "caution" were not kept in a locked place.

1. The Cuddle bug 2 classroom storage closet was unlocked containing multi-purpose solution, cleaners, and antiseptic.

2. In the School Aged area, the closet was unlocked containing multiple disinfectant and carpet cleaners, and rust remover accessible to the children in care.

3. The center's kitchen unlocked containing multiple disinfectant cleaners.
4. Photographs taken by the inspector of the areas containing the hazardous substances being unlocked during the inspection.

Plan of Correction: The center will ensure staff monitor door/storage areas containing hazardous substances are kept locked and inaccessible to children in care at all times.

Standard #: 8VAC20-780-340-F
Description: Based on observation and interview, the center did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence: During the inspection on May 18, 2022, in the Cuddle bug 2 group there were six children with the youngest being 14 months observed napping on their cots in the classroom without staff supervision. Interview with staff confirmed staff not being present in this room during this inspection.

Plan of Correction: The center will ensure staff provide sight and sound supervision of children in care at all times.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview with staff, the center did not ensure to follow the required staff to children ratios whenever children are in care:

Evidence: During the inspection on May 18, 2022, the inspector observed one staff present with nine children in the Cuddle bug 1 room with the youngest child age, 5 months. Interview with staff confirmed being out of ratio during this inspection.

Plan of Correction: The center hired staff and will make sure two staff are present in the baby rooms to ensure staff to child ratios are followed.
