Thank you for your time and assistance with the unannounced inspection conducted for the relocation request. The inspection was completed on May 23, 2022 The purpose of the inspections was to tour the building prior to care commencing at the new location.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-40-E Description: Based on observation and interview, the facility did not ensure the center's activities, services, and facilities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department
Evidence:
1. The license is currently issued for the facility located at 317 N. Lewis Street, Staunton, VA 24401. No children were in care at the facility on May 23, 2022.
2. The licensee submitted an initial application for an address change on January 26, 2022, and provided the last required documentation on May 5, 2022.
3. The administrator stated the center opened at 320 N. Central Ave., Staunton, VA 24401 on May 10, 2022 despite not having the license for the new location.
4. The inspector observed children in care on May 23, 2022 at 320 N. Central Ave., Staunton, VA 24402.Plan of Correction: Not available online. Contact Inspector for more information.
