Comments:
An unannounced monitoring inspection was conducted on 05/24/2022. There were 36 children, ages 7 months-5 years, 9 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in morning snack, free choice activities, learning time and during lunch. The inspector discussed the following with the provider: diapering, behavioral issues and toddler gates. A total of 5 children?s records, 5 staff records and 1 medication were reviewed. The inspector arrived for the inspection at 9:00 am and departed at 12:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review and discussion, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two.

Evidence:
1. The record for Child 1 did not contain documentation of additional immunizations once every six months for children under the age of two; Child 1 was determined to be 15 months old. The most recent immunization record was dated 11/10/2021.
2. The record for Child 2 did not contain documentation of additional immunizations once every six months for children under the age of two; Child 2 was determined to be 14 months old. The most recent immunization record was dated 07/23/2021.

Plan of Correction: Will tell the parents today.

Standard #: 8VAC20-780-270-A
Description: Based on observation and discussion with staff, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The large metal storage cabinet in the Infant Classroom is broken and cannot close properly. This creates a pinch point and a sharp edge that could snag skin. The cabinet is accessible to crawling infants.
2. There is a broken electrical outlet cover plate in the 13-15 month classroom that is sharp and could snag skin. The broken cover plate is low to the ground and is accessible to the children.
3. There is a large white shelving unit in the room used for rest time that is not anchored to the wall that could be pulled over onto children.

Plan of Correction: Will fix all of the items.

Standard #: 8VAC20-780-500-A
Description: Based on observations and discussion with staff, the center failed to ensure that staff shall wash their hands with soap and running water before and after a diaper change.

Evidence:
1. Staff 2 was observed changing a child?s diaper. Staff 2 did not wash their hands after completing the diaper change.
2. Staff 3 was observed changing 5 children?s diapers. Staff 3 did not wash their hands in between diapering each child.

Plan of Correction: Will have a staff meeting to retrain the staff.

Standard #: 8VAC20-780-500-B
Description: Based on observation and discussion with staff, the center failed to ensure that the diapering surface shall be cleaned with soap and water and sanitized after each use.

Evidence:
1. Staff 2 did not sanitize the diapering surface after changing a diaper.
2. Staff 3 did not properly sanitize the diapering surface after each diaper change. The sanitizing solution, bleach and water, was sprayed and wiped off within 30 seconds. To sanitize an object the surface of the item is sprayed with the solution and allowed to air dry on the surface for a minimum of two minutes.

Plan of Correction: Will retrain staff and post the diapering steps in the diapering area.
