Comments:
A complaint inspection was conducted on May 26, 2022 from 10:30 A.M. to 12:45 P.M. in accordance with another local agency. A complaint was received in the Office of Child Care Health and Safety (Valley) on May 25, 2022 regarding forbidden action and behavior guidance. The inspector review compliance in the areas of administration, staffing, supervision and programming. One child and one staff record was reviewed and three staff were interviewed.

There was no preponderance of evidence therefore the complaint is not valid. There was one violation non complaint related noted during the inspection and was documented on the violation notice issued to the program.

Description: Based on review of children's records, the center failed to ensure that an acceptable form of proof of the child's identity and age was reviewed.

Evidence:
1. Child #1's record did not contain an acceptable form of documentation of proof of age and identity.
2. Staff confirmed that the proof of age and identity was not an official document.

Plan of Correction: Administration will review acceptable documentation with staff who are reasonable for children's records.
